Why this sheet exists

The Qualiopi 2026 reform now requires the physical presence of the director at the audit and evidence of "lived" quality — not just written procedures. The auditor looks for dated traces, documented pedagogical adjustments, learner signatures and real intermediate reviews.

This sheet transforms what you already do informally (in-class monitoring, observations, remediation, learner feedback) into a document the auditor can read in 2 minutes, filed in each learner's record.

4 usage rules

  • → One sheet per learner, for the entire duration of the program.
  • → Filled in as the training progresses, not on the last day. That's what the 2026 auditor is looking for.
  • → Signed by the learner at each intermediate review — the signature proves they have seen their progress.
  • → To be kept for at least 3 years in the learner's file.

Header (1 time per sheet)

Identifies the organisation, training, framework, learner and reference trainer. To be filled in at the start of the program.

→Organisation — Full name of the training provider / CFA / GRETA
→Training — Exact title (Professional Title, Bac Pro, BTS, etc.)
→Framework used — RNCP code or official reference
→Period covered — Start date → end date
→Learner — Full name
→Reference trainer — Full name

Tracking table (1 row per session)

The heart of the document. One row per session or block worked on. Filled in as the training progresses, never reconstructed afterwards.

4-level acquisition scale

Intentionally simple. Too many levels = loss of reliability between trainers.

A

Acquired and autonomous

The learner performs without help or reminder.

B

Acquired with reminder

Correct performance with an occasional reminder from the trainer.

C

Being acquired

Partial performance, to be reworked in a dedicated session.

D

Not acquired

To be restarted from scratch, report to pedagogical coordination.

What the Qualiopi 2026 auditor is looking for

To check before the audit. 6 boxes = proof of lived quality required by the reform.

  • The sheet is filled in as training progresses — regular dates, not a single block at the end.
  • At least one pedagogical adjustment is documented somewhere on the sheet.
  • The learner has signed at least one intermediate review.
  • The final review lists levels by block + trainer's opinion.
  • The sheet is filed in the learner's individual record.
  • The sheet is readable in under 2 minutes by someone who doesn't know the training.
